What are the rules for fasting these days? What about if you are in a nursing home and you have an extraordinary minister of communion bringing you the eucharist?

The Code of Canon Law, number 919says that Catholics should, as a rule, abstain from all food and drink, except water and medicine, for a least an hour before receiving holy communion.
The sick and the elderly are not bound by this eucharistic fast.
